I confirm that all players have voluntarily agreed to participate in the Josh Alexander Cup. We understand that participation in this tournament involves risks associated with the sport of football, including but not limited to physical injury, and we accept these risks.

We agree to abide by all rules and regulations set out by the tournament organisers, including those related to player conduct, game rules and safety protocols.

I confirm that all players are physically fit and medically cleared to participate in the tournament. In the event of injury or medical emergency, I authorise the tournament organisers to administer necessary medical treatment to any team member.

I understand and agree that the tournament organisers are not responsible for any injury, loss or damage to persons or property that may occur as a result of participation in the tournament.

I have read, understood, and agreed to all the terms and conditions outlined in this Declaration of Consent. I am authorised to sign this document on behalf of the team named above and I acknowledge that this consent is binding on all team members.
